Important Telehealth Disclaimer

Rei Direct Cares provides virtual healthcare services only. All visits are conducted remotely via a HIPAA-compliant telehealth platform. Please read the following carefully before scheduling a visit:
Emergency situations: Rei Direct Cares is not an emergency service. If you are experiencing a medical emergency, chest pain, difficulty breathing, stroke symptoms, severe injury, or any life-threatening condition, call 911 immediately or go to your nearest emergency room.
Clinical discretion: All treatment recommendations, diagnoses, and prescriptions are based on the provider's clinical assessment and judgment. Not every visit will result in a prescription or a specific diagnosis. Treatment plans are individualized and evidence-based.
Telehealth limitations: Not all medical conditions can be evaluated or treated safely through telehealth. Your provider will advise you if in-person evaluation or specialist referral is needed.
No established relationship guarantee: A single telehealth visit does not establish a long-term patient-provider relationship. Ongoing care requires continued engagement and follow-up visits as clinically appropriate.
State licensing: Services are available only to patients physically located in Wisconsin or Colorado at the time of the visit, where the provider holds an active license.

Financial Agreement & Dispute Resolution

Before initiating any chargeback or payment dispute with your bank or card issuer, I agree to contact Rei Direct Cares directly at 608-658-2424 or [email protected] within 48 hours of my visit to allow for direct resolution.
Filing a dispute without first exhausting this process is a violation of this agreement, and I understand that I will not be entitled to a refund for services that have been rendered. Failure to follow this process may result in permanent discharge from the practice and referral to a collections agency for recovery of disputed funds.
I confirm that I am the authorized holder of the payment method used for this transaction and that payment was made with my full knowledge and consent.

Cancellation & No-Show Policy

Cancellation 24+ hours in advance
Full refund of any balance beyond the non-refundable booking fee, or the option to reschedule at no additional cost.
Cancellation under 24 hours
50% of the total visit fee will be charged. This fee is non-refundable, as that time has been exclusively reserved for your care.
No-show or missed appointment
The full visit fee will be charged. No-show fees are non-refundable. If an emergency arises, please contact us as soon as possible and we will do our best to accommodate you.
Late arrival
Late arrivals may result in a shortened visit or rescheduling at the provider's discretion.

Consent to Treatment — Rei Direct Cares

By booking an appointment with Rei Direct Cares, I voluntarily consent to receive telehealth evaluation and treatment services provided by a licensed, board-certified Nurse Practitioner via a secure, HIPAA-compliant platform.
I understand that visits are conducted virtually, that clinical decisions are based on information I provide, and that not all conditions can be treated through telehealth. I may be referred to in-person care when necessary. Prescriptions are issued at the provider's sole discretion.
I confirm that I have provided accurate and complete medical information and that I voluntarily agree to receive care through this platform. I understand this service is not for emergencies — if I am experiencing a medical emergency, I will call 911 or seek immediate emergency care in any life-threatening situation, as telehealth is not appropriate for emergencies.
